Coach Application- Please make sure your application along with the filled out CORI form and the signed Coaches Code of Conduct is sent to lenoxyouthbball@gmail.com or PO BOX 2082 Lenox, MA 01240 by October 28th.

Coach Code of Conduct *- All coaches of the LYBA sign this document to assure that their behavior is appropriate at all LYBA events in order to be a role model in displaying sportsmanship.

CORI Form *- coaches please fill this out with your coach's application and your code of conduct!
